Fund Raising Event Forms

For those who wish to sponsor a fund raising event on behalf of the Child Alert Foundation, we have created forms for you to download and return back to the CAF office. The forms are contained in a self extracting executable module in Microsoft Word Format and can be obtained by sending an email to the CAF office.

If there are any promotional items that you'd like to order from the Child Alert Foundation such as promotional pins, caps, patches, etc., please send an email to the CAF office indicating how many of each item you would like to provide. These items can be obtained from CAF to help promote your event.
